Health care is provided under a new program recently offered by Kaiser. This plan allows employees and their dependents to utilize the Kaiser facility OR go to an in-network doctor, OR go to the doctor of your choice. This is a PPO plan that is fully described on the following link (2007 Summary of Benefits). Tricorp will pay 50% of the Employees' monthly plan cost and an additional 25% of the family plan. Dental insurance is offered through our provider Denex. This is a plan that covers up to $1500 per ANNUAL maximum per person and $1000 lifetime Orthodontist benefit. This plan is being paid as an fully paid benefit so 100% of the premium is paid by us. Long and short term disability and Life Insurance are being offered through our provider, Guardian. It covers up to 60% of your salary up to $1000 per week. The Life benefit is $50,000. Tricorp will pay for 100% of this benefit for short term disability and life. Then pass on 50% of the LONG TERM disability to you so that 50% of the benefit received will be TAX FREE according to current tax regulations. The portion of premium that the employer pays is TAXABLE when you receive the benefit so this gives you a higher pay out in the event of long term disability. Tricorp also offers a 401 (k) savings plan available after six months of employment. Information will be sent to you prior to your eligibility directly from Putnam.
